PURPOSE/AIM

Multidisciplinary proposals for the significant revision of the pathological classification of lung adenocarcinoma have recently been published by the IASLC, ATS and ERS. We aim to:   Summarize the key changes in nomenclature and their underpinning rationale Illustrate their radiological appearances with pathological correlation  

CONTENT ORGANIZATION

Revision rationale (e.g. survival/treatment of subsolid lesions, molecular and therapeutic advances e.g. EGFR mutations) Classification changes (e.g. elimination of bronchioloalveolar carcinoma, introduction of adenocarcinoma in situ, minimally invasive adenocarcinoma) CT appearances of new entities (histological old/new term correlations) T staging implications (solid v ground glass), CAD, biopsy material Radiological follow-up: guidance, lesion evolution, multifocal lesions    

SUMMARY

The major teaching points are: Awareness of new integrative proposals based on the biology and evolution of subsolid lung adenocarcinoma. Abolition of BAC and introduction of precursor-type lesions which will require further TNM revisions. Interim recommendations to classify subsolid lesions, for radiological staging and observation, and an awareness of the contemporary nomenclature and its management implications will assist radiologists in providing more clinically relevant reports.
