Diameter of the Efferent Vein: An Accurate Marker for Detection of Pulmonary Arteriovenous Malformations Patency Recurrence after Embolotherapy

PURPOSE

In hereditary hemorrhagic telangiectasia (HHT), post-embolotherapy recurrence of Pulmonary Arteriovenous Malformations (PAVMs) patency is often difficult to assess with CT. We evaluated whether the measurement of the diameter of the efferent vein could be an accurate predictor for PAVMs patency.

METHOD AND MATERIALS

We studied 89 consecutive patients presenting with HHT and PAVMs who underwent at least two sessions of embolotherapy. The CTs performed systematically within 7 days prior to the last sessions were considered in this study. The diameter of the efferent vein of all the previously treated PAVMs were blindly measured and compared to the per- embolotherapy findings in term of PAVM patency. A ROC analysis was performed to determine the optimal predicting vein diameter.

RESULTS

50 over 176 analyzed PAVMs were recurrent. Mean efferent vein diameter was respectively 4.3+/-1.5 and 1.6+/-0.8 mm for the patent and non-patent PAVMs. Optimal cut-off diameter from the ROC analysis was 3 mm (sensitivity=93.5%; specificity=91.2%). A sensitivity of 100% was reached for a diameter larger than 2 mm.

CONCLUSION

Diameter of the efferent vein may be proposed as a marker for detection of PAVMs patency recurrence.

CLINICAL RELEVANCE/APPLICATION

Measurement of the diameter of the efferent vein predicts PAVMs patency recurrence.
