Clinical Outcome Assessment in Screening Ultrasonography (US): Medical Audit of Screening Breast US in Mammographic Negative Population

PURPOSE

To perform an audit of screening US, and to evaluate the performance of breast US as second-line screening test

METHOD AND MATERIALS

We analyzed 2370 screening breast US examinations (2306 patients) performed at our health promotion center for 1 year. All final assessments were based on the Breast Imaging Reporting and Data System (BI-RADS). A total of 1666 patients with at least 1 year follow up breast imaging (US and/or mammography) with negative screening mammography (n=1522) or screening breast US only (n=144) due to young age were finally enrolled in this study. We analyzed recall rate, cancer detection rate, sensitivity, specificity, PPV (PPV1, PPV2) and minimal cancer rate.  

RESULTS

The percentage of dense mammographic composition was 75.5% (1258/1666). In our study, the recall rate was 5.2 %. Sensitivity, specificity and positive predictive value (PPV1, PPV2) were 60%, 95.1% and 7.5% respectively. The additional cancer detection rate was 6/1000. Minimal cancer rate, percentage of stage I cancer and lymph node positivity were 50%, 80% and 10% respectively.

CONCLUSION

Breast US in mammographically negative population showed similar recall rate, relatively low sensitivity, but high specificity and PPV compared with desirable goals of screening mammography. Screening US will be useful second-line screening test to increase detection of more cancers at small size and low stage, especially in dense breast population.

CLINICAL RELEVANCE/APPLICATION

The clinical outcome of screening breast US in negative mammogram population is not well known. Our medical audit data of screening US will help the use of breast US as a second-line screening test.
