Evaluation of Application of MRI in the Diagnosis of Heart Failure Caused by Old Myocardial Infarction and in the Assessment of Therapeutic Effects of Stem Cell Transplantation

PURPOSE

To evaluate the application of MRI in the diagnosis of heart failure induced by old myocardial infarction (OMI) and in the assessment of therapeutic effects of stem cell transplantation.

METHOD AND MATERIALS

20 patients (male=18, female=2, age 59.5±10.1y) with OMI were evenly divided into 2 groups(group A: CABG+stem cell transplantation, group B :CABG only). preoperative LV functions were measured by MRI in patients with heart failure induced by old myocardial infarction. The changes of left ventricular functions after stem cell transplantation were evaluated by MRI 6 months after operation. 18F-FDG myocardial metabolism imaging and late contrast-enhanced MRI were employed to detect viable myocardium, and the accuracy of MRI was analyzed.

RESULTS

The LVEF was improved both in group A and B(mean 8.63% vs. 10.37%, P=0.451) .In group A,LVEDV was(9.91±39.50)ml , which was significantly different from that in group B [(-27.91±21.86)ml, P=0.024]. Ventricular wall thickening ratio were significantly different from that in group B (11.40%±11.53% vs. 2.27%±7.20%,P<0.05), while the LVESV, CO,CI, LV wall thickness didn’t show significant difference between the two group. The interpretation of late contrast-enhanced MRI had good agreement with 18F-FDG myocardial metabolism imaging SPECT in assessment of myocardial viability, kappa value was 0.446 (P<0.001). Compared with 18F-FDG myocardial metabolism imaging, which was taken as a gold standard, the sensitivity of late contrast-enhanced MRI was 68.3% and the specificity was 92.5%. The changes of segmental wall thickness were difference between stem cell treatment group and control group (P=0.044).

CONCLUSION

Stem cell therapy on top of CABG aggravated LV remodeling in heart failure patients with old myocardial infarction. The specificity of MRI is similar to (18)F-FDG SPECT while the sensitivity is inferior to (18)F-FDG SPECT on detecting viable myocardium.

CLINICAL RELEVANCE/APPLICATION

MRI can detect subtle changes in wall motion and thickness and is capable of being applied to the evaluation of early outcomes after stem cell transplantation.
