PURPOSE/AIM

Periapical lucency is a common “incidental” finding on CT, particularly in ER settings and is often underappreciated. The purpose of this exhibit is: 1. To review imaging findings of periapical lucency on CT 2. To discuss the differential diagnosis of periapical lucency 3. To discuss the clinical relevance of periapical lucency

CONTENT ORGANIZATION

1. Pathophysiology of periapical lucency 2. Review of CT findings 1) periodontal disease (periapical granuloma, periapical abscess and periapical cyst) 2) complication from periodontal infection; extraosseous infection, abscess 3) “incidental” periapical lucency causing symptoms/complications; cases referred as sinusitis, eye pain, etc. 4) trauma; tooth subluxation 5) odontogenic and non-odontogenic neoplastic and non-neoplastic lesions 3. Differential diagnosis of periapical lucency

SUMMARY

The major teaching points of this exhibit are: 1. Periapical lucency is very common finding in the head and neck CT, most likely representing periodontal disease, such as periapical granuloma, periapical abscess or periapical cyst. 2. Periapical lucency may be a totally incidental finding, however it can be a cause of condition referred for imaging and immediate intervention may be needed. 3. Familiarity with these lesions is important for all radiologists to improve patient care.
