Breast Keynote Speaker: Results of the ACRIN DMIST Trial

PURPOSE

Purpose: The American College of Radiology Imaging Network (ACRIN), which is funded by the National Cancer Institute, conducted the Digital Mammographic Imaging Screening Trial (DMIST) to compare the diagnostic accuracy of digital and screen-film mammography in asymptomatic women presenting for screening for breast cancer. Digital mammography was developed to address some of the limitations of traditional screen-film mammography.

METHOD AND MATERIALS

Methods: 49528 asymptomatic women presenting for screening mammography at 33 sites in the US and Canada were enrolled into the study. Subjects underwent both digital and film examinations. A total of 42760 (86.3%) subjects were eligible and had full reference standard information. Two different radiologists read the film and digital examination for each subject. Breast cancer status was determined by follow-up mammography 11 months or later after study entry or by breast biopsy information within 15 months of study entry.

RESULTS

Results and Conclusion: ROC performance will be presented for digital and film mammography across the entire eligible population and for each machine type, as well as for the subsets of the population according to breast density, age and menopausal status. Sensitivity, specificity and positive predictive values will be reported using a 7-point malignancy scale, the BIRADS® scale and radiologist’s recommendation for call back. Total numbers of cancers and the histologic types and stages of cancers detected by both modalities will be described.
