PURPOSE

To assess the safety and efficacy of MR-guided laser ablation of liver malignancies

METHOD AND MATERIALS

Between 01/2001 and 12/2003 we treated 108 patients with 191 liver malignancies with MR-guided laser ablation. We performed 156 treatment sessions (496 applicators). In 64,7% the primary tumour was colorectal carcinoma, in 9% breast cancer, in 6,4 % HCC and others in 19,9 %. We stratified in subgroups of tumours > 5 cm and < 5 cm (76 and 24%, respectively). Examinations based on MRI were performed two days after the treatment and then every three months.

RESULTS

The rate of clinically relevant complications was 2,6% and the 30 day mortality was 1,9 %. Median follow up was 8 months (min.=0,3; max.=33,8). Local tumour control after 6 months was 93,2%, after 9 months 84,8%. The median progression free survival was 5,35 months (min.= 0,3; max.= 27). Overall median survival from time of initial diagnosis was 61,7 months (95% CI: 44,8-78,5) and 25,1 months after treatment (95% CI: 19,8-30,4).

CONCLUSIONS

In patients with liver metastases, local tumour ablation by MR-guided LITT is effective with just minimal procedure related morbidity or mortality.
