MR-guided Biopsies of Retroperitoneal Space Occupying Lesions: Technique and Results

Abstract: HTML Purpose: To evaluate the safety and precision of MRI-guided biopsies of retroperitoneal space occupying tumors in an open low field system. Methods and Materials: In 30 patients with indistinct retroperitoneal or paraaortic tumors MR-guided biopsies were performed using a low field system (0.2 T, Magnetom Concerto, Siemens, Germany). For the monitoring of the biopsy-procedure T1-weighted FLASH sequences (TR/TE=100/9;FA= 70 degree) in all patients were used and additional modified FLASH-sequences in 18 patients. After positioning of the needle tip in the tumors 129 biopsy specimens were acquired with 16 G cutting needles (Somatex, Germany) during the interventional procedures. The size of the lesions ranged from 1 to 4 cm. The visibility of the needles and the precision of the biopsy procedure were evaluated. Results: The biopsies were performed successful in all patients without vascular or organ injuries. Adequate specimens for histological interpretation were obtained in 29 cases (96%). In 1 patient (4 %) the lesion was missed with MR-guidance and also with CT-guidance so that laparotomy was needed for histological clarification. The organs, tumors and vessels were easily determined with FLASH-sequences. T1-weighted FLASH images proved optimal for confirming needle-tip placement during the biopsies of paraaortic lesions. The visualization of the aortic blood flow with MRI facilitated the biopsy procedures of paraaortic lesions. Conclusion: MR-guided biopsies of retroperitoneal or paraaortic lesions using an open low field system can be performed safely and precisely with sufficient visualization of the lesions and the aorta.       Questions about this event email: Zangos@em.uni-frankfurt.de
