Diagnostic Value of Contrast-enhanced MR Angiography for Detection of Atherosclerotic Vascular Disease of the Supraaortic Arteries: Intraindividual Comparison with Digital Subtraction Angiography in 833 Arteries

Abstract: HTML Purpose: Prospective, intraindividual comparison of high spatial resolution contrast-enhanced 3D MR angiography (CE 3D MRA) and digital subtraction angiography (DSA) for the assessment of atherosclerotic vascular disease of the supraaortic arteries. Methods and Materials: CE 3D MRA with a random segmentation of central k-space and DSA were performed in 50 consecutive patients. Parameters for MR angiography were as follows: 1.5T Intera MR system (Philips Medical Systems, Best, NL); TR/TE = 4.8/1.48 ms; flip angle, 40; RFOV 50; 0.2 mmol Gd-DTPA per kg body weight @ 3 ml/sec. A high image matrix of 432 x 432 on a 350 mm FOV was used yielding a non-zerofilled voxel size of 0.81 mm x 0.81 mm x 1 mm (0.66 mm3). The FOV covered the supraaortic arteries from the aortic arch up to the circle of Willis and its main branches. From a total of 900 arteries (18 arteries in 50 patiens), intraindividual comparison of CE 3D MRA and DSA was available in 833 arteries. Atherosclerotic vascular disease of these 833 arteries was assessed independently by two radiologists according to the NASCET criteria. Results: CE 3D MRA had a sensitivity of 100% (73/73), a specificity of 99.3% (760/765), a PPV of 93.6% (73/78) and a NPV of 100% (760/760) by using a ³ 70% threshold of arterial diameter stenosis. For detection of occlusion, sensitivity, specificty, PPV and NPV of CE 3D MRA were 100%, respectively. Conclusion: High spatial resolution CE 3D MRA appears to be adequate to replace diagnostic DSA for the detection of atherosclerotic vascular disease of the supraaortic arteries.
