Purpose: To assess the clinical significance of Rauber¢s sign, the previously described radiographic sign of meniscal tear
Methods and Materials: MR images of 348 consecutive knees in 340 patients were retrospectively reviewed for meniscal tear and cruciate ligament injury. Anteroposterior radiographs were reviewed for Rauber¢s sign in medial and lateral tibial plateau, respectively. Positive Rauber¢s sign was defined as any projecting traction spur or periosteal deposition in tibial plateau. The presence of Rauber¢s sign on radiograph and diagnosis on MR for meniscal tear were analyzed statistically. The relationship between the presence of Rauber¢s sign and the duration of the knee injury were evaluated.
Results: Among the 348 knees, 111 knees had meniscal tear only, 70 had both meniscal and cruiciate ligament injury, and 51 had cruciate ligament injury only. Rauber¢s sign was seen on AP radiograph in 44% (49/111) of the patients with meniscal tear only, 54% (38/70) of the patients with both meniscal and cruciate ligament injury, and 6% (3/51) of the patients with cruciate ligament injury only. For diagnosis of meniscal tear, the sensitivity, specificity, positive and negative predictive values of Rauber¢s sign were 51%, 93%, 83%, 75% for the medial meniscal tear; and 29%, 95%, 62%, 54% for the lateral meniscal tear. The mean duration of the knee was 71 months in patients with positive Rauber¢s sign, and 20 months in those with negative Rauber¢s sign.
Conclusion: Rauber¢s sign on radiographs was proved to be a specific sign for the diagnosis of meniscal tear. The longer the duration of knee injury, the higher the prevalence of Rauber¢s sign was.
