Fluid within Tendon Sheaths of the Ankle and Associated Tendon Disorders: Study with MRI and Intravenous Injection of Contrast

Abstract: HTML Purpose: To evaluate the frequency of tendon abnormalities associated with fluid within the tendon sheaths of the ankle. Methods and Materials: 1.5 T MR images of 220 ankles referred from routine imaging between January 2000 and December 2002 were evaluated for the presence of fluid within the synovial sheath of the tendons. Enhancement of tendon sheath after intravenous (IV) injection of gadolinium (Gd) was evaluated. Associated tendon lesions were recorded. Results: In 98 ankles, fluid was found within tendon sheaths. Fluid was found within the flexor hallucis longus (FHL) tendon sheath in 42 cases, with an enhancement after IV of Gd in 2 cases, and was associated with fluid in the ankle, without abnormalities of the FHL tendon in all the 42 cases. Fluid in the sheath of the tibialis posterior tendon (TP) (n=19), was enhanced in 15 cases, with abnormalities of the tendon in 12 of the 15 cases and fluid in the tibiotalar joint in 4 cases. Fluid in the peroneal tendon sheath was found in 39 cases with enhancement in 27 cases and abnormalities of the peroneal tendons in 27 of the 30 cases. Lack of tendon sheath enhancement was not associated with tendinous lesion for the TP (n = 4), but was associated with tendinous lesion for the peroneal tendons (n=9). Conclusion: Fluid within the tendon sheath of the ankle is a common finding with MRI. Enhancement of the tendon sheath after IV of Gd is associated with tendon abnormalities for the TP and peroneal tendons. Conversely, lack of enhancement of the fluid within tendon sheath is not associated with TP abnormalities, and can be associated with peroneal tendons abnormalities.       Questions about this event email: fabienne.bach@cch.ap-hop-paris.fr
