The Value of Abdominal CT for the Diagnosis of Meckel Diverticulum

Abstract: HTML Purpose: To determine the CT findings associated with the presence of a diverticulum of Meckel and/or its complications, in order to improve the preoperative CT diagnosis. Methods and Materials: Medical and radiological records of 36 consecutive patients in whom the presence of a diverticulum of Meckel was asserted at surgery between January 1992 to December 2002 were analysed. Sixteen patients underwent an abdominal CT before surgery and were included in the series. Images were re-analysed to determine the CT findings which were the most frequently associated to the presence of a diverticulum of Meckel and/or its complication. Results: Abdominal CT was normal in 4 patients with diverticulum of Meckel (4/16, 25%) : at surgery, three of them had a normal appearance, the fourth was actively bleeding. CT was abnormal in 12 patients : 7 of them had inflamed Meckel diverticulum at surgery (3 of them were identified as diverticulitis of Meckel by the radiologist before surgery, 4 were diagnosed retrospectively on CT). In 3 cases, the diverticulum of Meckel was an incidental surgical finding and was found retrospectively on CT images. One bleeding diverticulum was correctly diagnosed at the admission CT. One diverticulum had a CT appearance of ileal intussusception, but could not be identified as a diverticulum of Meckel at retrospective analysis. The CT sensitivity for detection of a diverticulum of Meckel and/or its complications was 25% (4/16) at initial CT interpretation and 63% (10/16) at retrospective analysis. Conclusion: Our series suggests that the sensitivity of CT to detect a normal or pathological diverticulum of Meckel could be greatly improved by a systematic analysis of the characteristic CT findings.       Questions about this event email: alex1platon@yahoo.com
