Imaging Of Colonic Diverticulitis: Multimodality Approach, Controversies, And Future Directions

TEACHING POINTS

1. Suspected colonic diverticulitis (CD) is usually imaged with CT. CT can be used to accurately confirm or refute the diagnosis, to search for alternative diagnoses, and to identify a spectrum of potential severity and complications. Suspected or known CD can occasionally be imaged with other cross-sectional imaging modalities, including ultrasound, DECT and MRI.2. Acute CD can be complicated by perforation, peritonitis, abscess formation, fistula formation, small and/or large bowel obstruction, urinary tract obstruction, and colonic bleeding. Rare but important complications include septic thrombophlebitis, pseudoaneurysm formation in the abdominal aorta from associated septic seeding, and adnexal/uterine involvement.3. CT can be used to identify risk factors for progression from acute uncomplicated CD to complicated CD.4.Colonic malignancy can mimic CD and vice versa but there are imaging signs which may be helpful in differentiating between these diagnoses.
