TEACHING POINTS

 1. Primary bone tumors and metastatic bone involvement  accounts for significant mortality and morbidity. 2. Early detection is difficult, as the majority of patients are asymptomatic. Symptoms include bone pain, deformity, compression of the soft tissues and pathological fractures. 3. Lesions can be lytic, sclerotic or mixed. 4. Bone scan is sensitive routine imaging modality used to identify bone lesions, and SPECT improves the detection. 5. Positron emission tomography (PET) using 18F-fluoride (18F) or 18F-fluorodeoxyglucose (18FDG) also aid in  detecting osseous metastasis. 6. 18F-Fluoride PET/CT is a sensitive and specific modality for detection of bone metastases, and correlation  with non-optimized CT portion increases the sensitivity.

1. Introduction to nuclear medicine techniques used in the evaluation of osseous lesions 2. Tracer principle behind Tc-99m MDP bone scan (planar and SPECT), F-18 FDG PET and F-18 Fluoride PET, as well as special considerations/limitations per agent will be discussed. 3. Imaging characteristics of lytic, sclerotic , mixed metastases, and metastatic malignancies with osseous metastasis predilection. 4. Differentiating recurrence vs post radiation changes and benign conditions including secondary marrow activation, degenerative disease and incidentalomas will also be described.
