1. Diffusion-weighted imaging(DWI) has been used as a rapid and useful diagnostic tool for detection of acute cerebral infarction (ACI) which shows high signal intensity(SI).
2.But in some cases, high signal intensity on DWI does not always indicate the ACI. We will present many diseases that show high SI on DWI, mimics ACI and suggesting differential clues.
High DWI lesion mimicking acute cerebral infarction
1. Hemorrhage (T2 or T1 shine-through effect) : subacute hemorrhage, chronic hemorrhage, cortical laminar necrosis, diffuse axonal injury
2. Acute encephalitis : herpes simplex encephalitis(HSE), japanese encephalitis(JE), enteroviral meningoencephalitis, AIDS encephalopathy, creutzfeldt-Jakob disease(CJD)
3. Acute demyelinating disease or vasculitis : acute multiple sclerosis, SLE-vasculitis, anti-phospholipid antibody syndrome, neuro-bechet`s disease
4. Acute metabolic or toxic encephalopathy : hypoglycemic, hyperglycemic encephalopathy, acute and delayed CO intoxication, hypoxic-ischemic encephalopathy, acute ODMS, Wernicke encephalopathy, uremic encephalopathy, toxic encephalopathy
5. Benign or malignant mass lesions : abscess, epidermoid cyst, malignant tumor
6. Others : acute Wallerian degeneration, seizure-related acute hemispheric change
