1. Pathogenesis and advances in genetics: High-grade serous ovarian cancer originates in the fimbria and is associated with p53 mutations compared to low grade that is associated with BRAF or erb-B2 mutations.
2. 2014 FIGO staging update: Positive retroperitoneal nodes moved to IIIA and IVA defined as pleural effusion with positive cytology, among other changes.
3. Metastatic pattern: Peritoneum, lymph nodes and liver common; thoracic (pleura, lymph nodes) uncommon; rarely brain metastases
4. Treatment options:
a. Surgical options: Neo-adjuvant, adjuvant, cytoreduction, second look laparotomy
b. Conventional platinum-based agents- intravenous, intraperitoneal
c. Newer options- Anti-angiogenic agents (bevacizumab, cediranib), PARP inhibitors (olaparib), anti-HER2 agents (trastuzumab, pertuzumab), anti CA125 agents (oregovomab)
5. Complications:
a. Intraperitoneal chemotherapy- Intestinal obstruction
b. Anti-angiogenic drugs- Thromboembolic complications, hemorrhage, pneumatosis and intestinal perforation.
c. Anti-HER2 agents- LV dysfunction.
