• Medical errors are usually the result of several combined factors, and these factors can be grouped into several categories: Institutional, organizational, work environment, team environment, staffing, task-related, and patient characteristics. • Root Cause Analysis is a retrospective approach used to get to the “Root Cause” of the problem, which when identified and resolved, prevents recurrence of the problem. RCA is designed to help identify not only what and how an event occurred, but more importantly why it happened. • This exhibit will explain root cause analysis so that the reader can understand and perform the processes on their own.
