On MR, abnormal signal in bone marrow has wide differential diagnosis
Recognition of pattern to signal abnormality helps to narrow differential diagnosis
The common patterns of bone marrow abnormality are subtendinous, subligamentous, subchondral, subphyseal, endosteal, periosteal, kissing bone contusions, double line sign, penumbra sign, fat containing, fluid-fluid level, and multiplicity
Marrow conversion / reconversion
Patterns of bone marrow abnormalities and associated differential diagnosis
Subtendinous: tendon tear, tenosynovitis, HADD, enthesitis
Subligamentous: ligament tear, renal osteodystrophy
Subchondral: chondromalacia, OCD, kissing bone contusions, insufficiency fracture, AVN, transient osteoporosis, infection, inflammatory arthritides, neoplasms
Subphyseal: SCFE, Brodie's abscess, renal osteodystrophy, rickets, metastases
Endosteal: stress fracture, osteoid osteoma, osteomyelitis
Periosteal: stress fracture, osteoid osteoma, osteomyelitis
Kissing bone contusions: clip injury, dashboard injury, hyperextension injury, patellar dislocation, pivot shift injury
Double line sign: bone infarct
Penumbra sign: Brodie's abscess
Fat containing: intraosseous hemangioma, intraosseous lipoma
Fluid-fluid level: benign or malignant
Multiplicity: "FEEMMHI" mnemonic
