Comparison of Single and Double Dose of Contrast Media in Late Gadolinium Enhancement Magnetic Resonance Image of Myocardial Infarction

To compare the contrast with the use of single dose of gadolinium contrast agent in late gadolinium enhancement images in myocardial infarct with conventional double doses, in turbo FLASH inversion recovery(TFLIR) and phase sensitive inversion recovery(PSIR).
In 37 consecutive patients, late gadolinium enhancement images using both TFLIR and PSIR were examined. The patients were divided into 2 groups. 0.1 mM/kg and 0.2mM/kg of gadolinium-DTPA were injected to Group SD(N=17) and Group DD(N=20), respectively. Optimal inversion time was determined by TI scout in individual patients. The signal intensities(SI) and image noise were measured at the infarcted myocardium, normal myocardium and left ventricle(LV) cavity, and the contrast to noise ratio(CNR) were compared.
The average CNR between normal and infarcted myocardium in SD group vs. DD group were 15.46± 20.7 vs. 40.39±16.1(p<0.05) in TFLIR and 9.47±2.77 vs. 11.18±2.67 in PSIR(p<0.05). The average CNR between infarcted myocardium and LV cavity in SD group vs. DD group were 7.73±10.7 vs. -6.6±19.0(p<0.05) in TFLIR and 4.12±2.33 vs. -0.35±4.10 in PSIR(p<0.05). The DD group showed better CNR between normal and infarctd myocardium. But the CNR between infarcted myocardium and LV cavity was higher in SD group. The SI of the LV cavity to infarcted myocardium was higher in DD and lower in SD. The SI difference of the LV cavity to infarcted myocardium in DD in PSIR was smaller than TFLIR.
The CNR between infarct and normal myocardium was higher in DD group in both TFLIR and SIR, but the differences were smaller in PSIR. Meanwhile, the best CNR between infarct myocardium and LV cavity was seen in SD group and PSIR sequence image.
The use of single dose of contrast agent could help determining subendocardial border of infarcted myocardium, on late gadolinium enhancement MR.
