Detection and Characterization of Breast Lesions: Comparison between One-view Digital Breast Tomosynthesis and Two-view Full Field Digital Mammography

PURPOSE

To compare the ability of one-view digital breast tomosynthesis (DBT) and two-view full field digital mammography (FFDM) for the detection and characterization of breast lesions in a diagnostic population.

METHOD AND MATERIALS

317 paired examinations including 363 verified cancers were randomly reviewed by three experienced radiologists. Each examination consisted of one-view (mediolateral oblique) DBT, and two-view (craniocaudal and mediolateral oblique) FFDM. The detection of each lesion was compared in DBT and FFDM, and subclassified according to breast density and tumor invasiveness. The characterization of each lesion (BI-RADS) and seven-level probability-of- malignancy scores were compared in DBT and FFDM. For statistical analyses, the McNemar test and 2x2 tables were used. For binary outcome, classification of BI-RADS category 4 or higher was defined as positive for cancer.

RESULTS

Overall lesion detection rate was higher for DBT (316/363, 87.1%) than for FFDM (300/363, 82.6%). The detection rate for DBT was significantly higher than for FFDM in dense breasts (P = 0.004) and invasive tumors (P = 0.036), but was not different in non-dense breasts (P = 1.000) and non-invasive tumors (P = 0.125). Among the detected lesions, a BI-RADS score greater than or equal to 4 was assigned in 308/316 (97.5%) for DBT and in 291/300 (97.0%) for FFDM. For detected lesions, the probability-of-malignancy score was significantly higher for DBT than for FFDM in both dense (P < 0.001) and non-dense breasts (P = 0.022).  

CONCLUSION

One-view DBT is comparable to two-view FFDM for the detection and characterization of breast lesions in the diagnostic population. In dense breasts, one-view DBT is superior to two-view FFDM for detection with a greater level of confidence.  

CLINICAL RELEVANCE/APPLICATION

One-view DBT is useful for the detection and characterization of breast lesions in the diagnostic population, especially patients who have dense breasts.
