Sensitivity of early ischemic lesion identification in noncontrast CT (NCCT) scans is low. We propose an automatic method for rapid identification, localization and estimation of early (<4.5h) acute ischemic infarct region in NCCT to complement and potentially match or even outperform neuroradiology stroke expert.
Our RSNA2010 exhibit is extended with a better algorithm enhanced with integration of sulcal effacement analysis and basal ganglia atlas. The method segments grey/white matter, ventricles, cortical CSF, and analyzes changes in intensity distribution across hemispheres and individual structures to rapidly identify and localize infarcts as well as to estimate their volumes without infarct segmentation. The method will be illustrated by a CAD system with about 200 NCCT cases with follow-up imaging.
This exhibit 1) presents a stroke CAD able to automatically and rapidly localize early infarcts in NCCT; 2) localizes the infarct in axial, coronal, sagittal planes; 3) estimates infarct volume; 4) outperforms novice and often general radiologist, which makes it useful in Emergency Department; 5) is able sometimes to outperform expert when infarct is not visible to human eye in hyperacute stage; and 6) provides a solution which potentially increases sensitivity of stroke identification in NCCT.
