PURPOSE/AIM

To illustrate DWI findings of various CNS pathological conditions associated with intramyelinic edema and discuss the pathology and pathophysiology correlated with the distribution and reversibility

CONTENT ORGANIZATION

We will organise various CNS diseases with intramyelinic edema according to etiologies and discuss the reversibility and distribution on MRI and DWI correlated with pathology including electron microscopy. Etiologies included vigabatrin-associated vacuolar myelinopathy, spectrum of reversible splenial lesion of corpus callosum, osmotic myelinolysis, Wallerian degeneration, metabolic and toxic leukoencephalopathies, (phenylketonuria, Canavan disease, metachromatic leukodystrophy etc.), acute cytotoxic plaque of multiple sclerosis, acute disseminated encephalomyelitis, progressive multifocal leukoencephalopathy, chemotherapy-induced leukoencephalopathy, heroin-induced encephalopathy, delayed postanoxic leukoencephalopathy, and diffuse white matter changes after traumatic diffuse axonal injury and cerebral fat embolism.  

SUMMARY

Intramyelinic edema is considered one of the plausible explanations of complete reversibility of diffusion restricted edema. We illustrate the intramyelinic edema in various pathological conditions and discuss the pathology and pathophysiology correlated with its distribution and reversibility.
