TB or Not TB, That Is the Question: Central Nervous System Tuberculosis and Its Mimics

PURPOSE/AIM

Tuberculosis (TB) has a variety of central nervous system (CNS) radiologic appearances that can overlap with other diseases, which may present diagnostic challenges. We will provide a pictorial review of the different presentations of TB within the CNS, and review its mimics in terms of other disease processes.

CONTENT ORGANIZATION

1. Introduction to and pathophysiology of CNS TB. 2. Multi-modal imaging review of the following CNS TB lesions: tuberculous meningitis, base of brain abscesses secondary to prior healed meningitis, hematogenous tuberculous abscesses, tuberculomas of the spinal cord, and Pott’s disease of the spine. 3. Examples of CNS TB Mimics: Coccidioidomycosis, eosinophilic granuloma, bacterial abscesses, toxoplasmosis, cryptococcus, lymphoma, and sarcoidosis. 4. Approach to the differential.  

SUMMARY

Tuberculosis has a variety of CNS radiologic appearances that can be mimicked by numerous other disease entities. Familiarity with the various radiologic manifestations of TB and its mimics will allow for early diagnosis and timely initiation of appropriate therapy.
