Assessment of Tumor Hypoxia Using F-18 fluoromisonidazole PET in Newly Diagnosed Gliomas and Correlation of F-18 fluorothymidine PET

PURPOSE

The purpose of this study was to evaluate the uptake of F-18 fluoromisonidazole (FMISO) in newly diagnosed gliomas and the relationship to the uptake of F-18 fluorothymidine (FLT).

METHOD AND MATERIALS

A total of 21 patients with newly diagnosed gliomas were examined with FMISO PET and FLT PET before surgery. There were 4 low-grade gliomas (all grade II) and 17 high-grade gliomas (4 grade III and 13 grade IV). FMISO PET was scaled to the blood FMISO activity to create tumor to blood (T/B) images. Hypoxia was defined as a region with T/B greater than 1.2. FLT uptake was semiquantitatively assessed by using the standardized uptake value (SUV) for tumor (T) and contralateral normal brain tissue (N) by region-of-interest analysis. FLT T/N ratio was determined.

RESULTS

FMISO uptake was observed in all high-grade gliomas but not in low-grade gliomas. In 3 of 4 grade III gliomas, FMISO PET showed mild uptake with a maximal FMISO T/B < 2.0. In 12 of 13 grade IV gliomas, FMISO PET showed intense uptake with a maximal FMISO T/B ≥2.0. FLT uptake was observed in all high-grade gliomas but not in low-grade gliomas. In 3 of 4 grade III gliomas, FLT PET showed mild uptake with an FLT T/N ratio < 4.0. In all grade IV gliomas, FLT PET showed intense uptake with an FLT T/N ratio ≥5.0. In all low- and high-grade gliomas, there was a correlation between FMISO and FLT uptake within individual tumors, although there was a difference between the areas of FMISO and FLT uptake.

CONCLUSION

In this preliminary study, there was a correlation between FMISO and FLT uptake and glioma grade. In newly diagnosed gliomas, there was a correlation between FMISO and FLT uptake, despite a difference in their areas of uptake.

CLINICAL RELEVANCE/APPLICATION

In newly diagnosed gliomas, there was a correlation between FMISO and FLT uptake, despite a difference in their areas of uptake.
