Determination of Skeletal Development with MRI of Hand/Wrist in Celiac Disease after the Gluten-free Diet

PURPOSE

to establish the outcome of the gluten free diet on the skeletal development and growth in children with a known diagnosis of Celiac Disease (CD): an MRI of hand/wrist approach

METHOD AND MATERIALS

We recruited 45 subjects (4 to 12 y.o. 25 F, 20 M) with a proven diagnosis of CD. A low field open magnet (0.2 T) was used for this study. Informed consent was signed by tutors/parents after the approval of ethical board of our Institution. A single sequence T13D lasting 1° 39” with a slice thickness of 1.3 mm was the imaging protocol. Because “normal” standards of bone age like that of Greulich and Pyle are not available with MR, we assumed that each subject can be standard of himself. The known steps of bone maturity were used to compare the images before and after the gluten free diet with a follow-up of 6-12 months. The appearance of cartilage, vacuolization of cartilage, provisional calcification, progression of ossification, complete ossification were considered as they can be seen at MRI to detect progression of skeletal development.

RESULTS

38 patients showed a definite progression of bone age together with improvement of clinical situation, evaluated by pediatricians. 7 patients that did not present clinical improvement, showed similar images before and after the diet

CONCLUSION

MRI of hand/ wrist can be used to follow-up the outcome of the gluten free diet without a reference atlas of bone age

MRI of hand/ wrist can be used to follow-up the outcome of the gluten free diet without a reference atlas of bone age.
