Magnetic Resonance-guided High Intensity Focused Ultrasound Therapy: Clinical Experience of Single Center in the Treatment of Invasive Breast Cancer

PURPOSE

To evaluate the efficacy of non-invasive high intensity MR guided focused Ultrasound (MRgFUS) treatment for biopsy-proven invasive ductal breast cancer (IDC) (stage T1 M0 N0) scheduled to surgical resection and sentinel lymph node biopsy.

METHOD AND MATERIALS

Nine patients, with unifocal biopsy-proven IDC less than 2 cm in size and with confirm of an enhancing lesion in a treatable location at breast MRI exam, underwent ambulatory single session MRgFUS treatment using the ExAblate 2100 system (InSightec), under IRB approval. Patients scheduled and consented to surgical (lumpectomy or mastectomy) resection and sentinel lymph node biopsy. Pathology evaluated the efficacy of the treatment after surgery.

RESULTS

All 9 lesions were treated; in all patient no enhancement was seen at breast MRI after treatment and pathology demonstrated the absence of residual cancer after surgical excision, in all but 1 case (15% of residual tumor). Necrotic tissue with at least 5 mm margins of normal breast tissue around the necrosis was observed. We obtained excellent agreement between post-treatment MRI and pathology.

CONCLUSION

MRgFUS is a promising noninvasive treatment modality for unifocal breast cancer.

CLINICAL RELEVANCE/APPLICATION

MRgFUS is an innovative incisionless technique to obtained reliable ablation of invasive breast cancer without scar.
