Trochlea Dysplasia: Association with ACL Injuries

PURPOSE

To examine the correlation between trochlea dysplasia and internal derangement of knee.

METHOD AND MATERIALS

A retrospective review of 95 consecutive MRI of patients who were referred from a specialist knee clinic for internal derangement of knee at a teaching hospital was performed. The scans were reviewed by two experienced Musculoskeletal Radiologists using PACS (Picture archiving and communication system) and Computerized Radiology Information system (CRIS). Trochlea Dysplasia was classified using the Dejour system. The scans were reviewed and the following features were documented, cruciate injury, meniscal tears, chondromalacia patellae and patellofemoral joint osteoarthritis. 

RESULTS

58 subjects had dysplastic trochlea with 38 having Dejour type A. 13 subjects had ACL injury,3 had PCL injury and on subject had rupture of both cruciate ligaments. There was no ACL injury in control (normal) group. 11 subjects has medial meniscal tear with all being Dejour type A.

CONCLUSION

There is significant correlation between ACL injury and Dejour trochlear dysplasia especially type A. We hypothesize that people with Dejour type A trochlea dysplasia may be predisposed to ACL injuries.
