Uterine Artery Embolization for the Treatment of Obstetric Hemorrhage in Various Conditions

PURPOSE

To evaluate the role of uterine artery embolization (UAE) in the treatment of obstetric hemorrhage.

METHOD AND MATERIALS

All patients who underwent UAE for obstetric hemorrhage from March 2008 to December 2010 were included. Clinical success was regarded as obviation of hysterectomy. Patients’ demographics, clinical condition and procedure related radiologic findings were evaluated.

RESULTS

Forty-four procedures were performed in 41 patients (mean age, 33 years, range, 22-44). There were uterine atony in 12 patients, abnormal placentation in 8, and birth canal injury in 6. Initial clinical success was achieved in 32 (78%) patients. Second embolization which performed in three patients (2 uterine atony, 1 post D&C due to partial mole) who did not achieve sufficient hemostasis with first embolization was all successful. Hysterectomy was performed in six patients (3 abnormal placentation, 2 uterine atony, 1 post D&C due to scar pregnancy) who continue the bleeding despite of UAE. No major complication was occurred.

CONCLUSION

UAE is effective treatment modality to prevent hysterectomy, however, still limited success rate in patient with abnormal placentation.

CLINICAL RELEVANCE/APPLICATION

UAE is effective treatment modality to prevent hysterectomy in patients with obstetric hemorrhage.
