Computed Tomography vs Transthoracic Echocardiography for Diagnosing Congenital Heart Diseases: A Meta-Analysis

PURPOSE

To perform a meta-analysis to compare the diagnostic performance of computed tomography (CT) and transthoracic echocardiography(TTE) for the diagnosis of congenital heart diseases.

METHOD AND MATERIALS

MEDLINE, EMBASE, Cochrane library and some other databases, from January 1966 to October 2010, were searched for initial studies. All the studies published in English or Chinese relating to the diagnostic tests of TTE, CT,or both for congenital heart diseases were collected. Prospective and retrospective studies were included if they separately reported the rate of true-positive, true-negative, false-positive,and false-negative diagnoses of CHD from TTE and CT findings compared with the surgical results. Methodological quality was assessed by three readers. The statistic software called “Meta-Disc 1.4” was used for data analysis. A covariate analysis was used to evaluate the influence of patient or study-related factors on sensitivity.

RESULTS

In 15 included studies, CT had higher pooled sensitivity[0.95(95% CI: 0.94–0.96)] than TTE [0.87(95% CI:0.85–0.88)] in CHD.TTE had higher sensitivity[0.96(95% CI:0.94–0.97)] for cardiac malformation,but lower sensitivity[0.78(95% CI:0.76–0.81)] for extracardiac malformation.The AUC of CT and TTE were 0.1587 and 0.2043, respectively.The pooled sensitivity and AUC showed statistical significance between CT and TTE.

CONCLUSION

CT can provide added diagnostic information compared with TTE in patients with CHD, especially for patients suspected with extracardiac malformation.

CLINICAL RELEVANCE/APPLICATION

Both CT and TTE can provide useful diagnostic information for patients with CHD, and CT can provide addedl diagnostic information  for patients with extracardiac malformation.
