Colorectal metastasis (CRM) present with biliary invasion on CT in 10% of cases. This pattern of spread is often unrecognized ,compromizing staging and surgical management. Biliary invasion in CRM also has distinctive patterns of reccurence. We review imaging criteria at presentation, patterns of reccurence, pathological and surgical correlation
We review patterns of biliary invasion in colorectal metastasis. These findings are subtle and frequently overlooked on CT. Classification of these findings based on the level of biliary involvement as peripheral or central and further organized on the presence or absence of parenchymal mass is proposed.Distinctive patterns of recurrence after resection are described.
Clinical significance of these findings from the perspective of diagnosis, surgical resection and follow up are emphazised.
Surgical resection has lead to significant improvement in survival in CRM to liver. But unrecognized biliary invasion might significantly impact prognosis for patient with CRM. We present the spectrum of cross sectional imaging patterns of biliary invasion by CLM to increase awareness to this strategically important presentation.
