Role of Ultrasound for Evaluation of Post MTP (Medical Termination of Pregnancy) Sequele: Experience at Indian Rural Imaging Setup

PURPOSE

To diagnose uterine ,adenexal ,POD and abdominal signs in post MTP acute abdomen and specify the importance of ultrasound signs  To quantify and categorize Post MTP events in rural Indian population and to help clinician to manage the cases effectively  

METHOD AND MATERIALS

Transabdominal or transvaginal ultrasound was performed in 312 cases who presented with pain or bleeding PV within 72 hrs after Medical termination of pregnancy .Phillips HD 9 state of art ultrasound system was used with image grabbing and video cineloops preservation protocol based study.Imaging findings were used to grade the problem in every case under study. The set of findings were informed to clinician of center who after disscussion with team decided the treatment protocol for the same.

RESULTS

Spectrum Ultrasound findings were endometrial collection or POC, uterine rents ,bowel dilated nonperistaltic segments and pelvic collection. Incomplete evacuation was commonest (17) in whom products were removed .Uterine perforation of variable degree seen in 22 cases , 16 of which required surgical procedure (LAPROSCOPY 4 AND LAPROTOMY 12).In bowel and bladder injury suspected cases surgery was performed in 3 and 2 cases respectively. Ultrasound had sensitivity of 93% and specificity of 88% in diagnosing anatomical nature of problem and quantifying it.  

CONCLUSION

ULTRASOUND, helps to quantify and categorize problems. It plays key role in decision making process of intervention.TVS findings guide to decide regarding type of surgical intervention also ( LAPROTOMY vs LAPROSCOPY).Timely interventions would save life.In few situations intervention were easily avoided. Hence TVS must be used freely in post MTP cases. ULTRASOUND guide needs to be used in specific conditions like ,bicornuate uterus ,large fibroid and pregnancy ,IUCD and pregnancy .

CLINICAL RELEVANCE/APPLICATION

Ultrasound should be used in every Post MTP acute abdomen case as it helps in decision making of treatment options for clinicians.
