To highlight the similarities and differences of the imaging findings of CNS lymphoma in immunocompetent and immunocompromised patients.
We examined 44 patients with CNS lymphoma between the years of 2000-2007. The patients included in the analysis had at least an initial preoperative or pretreatment gadolinium enhanced MRI of the Brain. Of these 34 patients were presumed immunocompetent with no other sites of lymphoma involvement. 7 of the patients were immunocompromised secondary to positive HIV status and 3 were immunocompromised due to a history of organ transplantation.
Imaging characteristics were examined to show the qualitative differences between immunocompetent and immunocompromised patients. The most striking difference was the enhancement pattern: Peripheral enhancement was noted in 8/34 (23%) of the immunocompetent patients and all of the 10 immunocompromised patients. The other characteristic imaging patterns of primary CNS lymphoma will be reviewed to highlight the similarities and differences between the different catogories.
