Cervical Involvement by Endometrial Carcinoma: Assessment with DWI at 3 T

PURPOSE

 to investigate the value of diffusion-weighted MR imaging in detection of cervical involvement by endometrial carcinoma

METHOD AND MATERIALS

 Review the DWI of 26 patients with cervical involvement(CI) by endometrial carcinoma and 26 patients without CI as the control group. All the patients were confirmed by the surery and the histological data. The apparent diffusion coefficient (ADC) values were measured in the regions of interest within the intrauterine endometrial cancer, the internal os and the external os of the cervix of each patient. ROC analysis was employed to investigate whether ADC values could help in discrimination the cervical involvement.

RESULTS

 In the CI group, there were 15 patients with FIGO stage Ⅱ, 5 with Ⅲa, 4 with Ⅲc and 2 with Ⅳ. In the control group, there were 6 patients withⅠa, 13 withⅠb, and 7 with Ⅰc. ROIs of the external os could not be obtained in four patients of the control group because of the susceptibility artifact. In the patients with cervical involvement group, Both the mean ADC value of the intrauterine endometrial carcinoma((0.904±0.284)×10-3 mm2/s) and that of the the internal os cervix((0.924±0.237)×10-3mm2/s) were significantly lower than that of the external os of the cervix((1.205±0.403)×10-3mm2/s). While, there’s no significant difference between the intrauterine cancer and the internal os cervix. In the control group, the mean ADC value of the intrauterine endometrial carcinoma was (0.828±0.207)×10-3 mm2/s, and significantly lower than both that of the internal os cervix ((1.316±0.378)×10-3mm2/s)and that of the external os of the cervix. The mean ADC values of the internal os cervix in the CI group is significantly lower and that of the control group(t=-4.842, P=0.000). While, the mean ADC values of the intrauterine endometrial carcinoma had no significant difference between the two groups(t=0.736, P=0.466). The AUC is 0.863 ,the optimal ADC threshold value is 1.27×10-3mm2/s, the sensitivity and the specificity were 92.3% and 57.7%.    

CONCLUSION

 ADC measurement can provide useful information in detecting the cervical involement by endomentrial carcinoma.

CLINICAL RELEVANCE/APPLICATION

 Cervical invasion by ECcan be detected by DWI, and measuring the ADC value of the internal os of the cervix is recommended as it can provide more accurate preoperative staging.
