PURPOSE/AIM

1. Review anatomy, function and pathways of the hippocampus and the Limbic system with correlation to conventional MRI, fMRI and DTI. 2. Review differential diagnosis of unilateral and bilateral hippocampal lesions and pathophysiology. 3. Present MRI examples and discuss important features correlated with clinical presentation.

Limbic system anatomy and function •Limbic Lobe, Associated nuclei, Fornix and circuit of Papez •Vascular supply Evaluation of Limbic system pathology Bilateral hippocampal atrophy: •Alzheimer disease, Multiple sclerosis •Chronic infarcts and ischemia Hippocampal signal abnormality: •Mesial temposral sclerosis, Glial neoplasm •Herpes encephalits, Limbic encephilitis •Acute infarct in anterior choroidal artery Mammilary bodies hyperintensity: •Wernicke encephalopathy  

SUMMARY

Knowledge of the limbic system anatomy is instrumental in recognizing and understanding the pathology involving the hippocampus and its clinical manifestations. Advanced MR imaging techniques and PET can assist in evaluation of the limbic system. The hippocampus and the limbic system are affected by a variety of degenerative, infectious, vascular and neoplastic conditions which can be differentiated with careful analysis of MR imaging and correlation with clinical features.
