Non-bronchial Systemic Arterial Embolization in Management of Massive Hemoptysis Secondary to Post-Tuberculous Bronchiectasis in Egypt

PURPOSE

To demonstrate the efficacy of bronchial and non bronchial systemic arterial embolization in the control of massive hemoptysis due to post-TB bronchiectasis and to present some interesting cases of aberrant non-bronchial systemic arterial supply to the lesions.

METHOD AND MATERIALS

We recruited cases of Post-TB bronchiectasis presenting with massive hemoptysis to chest dept., Assiut University Hospital, Egypt between Jan 2008-Jan 2010. Every patient underwent urgent clinical examination, chest x ray, chest CT and diagnostic and interventional angiography. Results: We recruited 36 patients (25 males and 11 females). Complete control of hemoptysis was achieved in all patients. The total number of non-bronchial systemic artery was 16/36 (44.4%) in this study. Details are shown in the table.  

RESULTS

CONCLUSION

Bronchial and non bronchial arterial embolization is a safe and effective non surgical treatment for massive hemoptysis due to post-TB bronchiectasis  . Non-bronchial systemic arterial supply of the lesions was 44.4% . So, we recommend that it should be searched for thoroughly especially if recurrence or failure of control of hemoptysis is seen after bronchial arterial embolization.  

CLINICAL RELEVANCE/APPLICATION

Bronchial artery embolization  is effective In the majority of cases, the bronchial artery is the source of bleeding. However, in some racases, the source is from non-bronchial systemic arteries.
