Radiofrequency Ablation of Soft Tissue Desmoid Tumors

PURPOSE

Desmoid tumors or aggressive fibromatosis are infiltrative and locally aggressive neoplasms. Although metastatic potential is negligible, recurrence after surgical resection is common. Pharmacotherapy and radiation are utilized in cases where surgical resection may not be feasible. We have used radiofrequency ablation (RFA) in the treatment of selected cases of desmoids tumors.

METHOD AND MATERIALS

Between 10/2003 and 04/2009, seven patients were referred to our musculoskeletal interventional clinic for radiofrequency ablation of known desmoids tumors. Three cases were excluded due to close proximity of major nerves. Five lesions in 4 patients were deemed amenable to radiofrequency ablation. Procedures were performed under CT fluoroscopy guidance. General anesthesia was administered. A flexible 10 cm radiofrequency probe (RITA Medical Systems, Inc.) was used and lesions were ablated for 10 minutes at 105 degrees C.  

RESULTS

Five lesions in four patients were ablated. Patients were discharged on the same day following the procedure. Post-procedural pain was mild to moderate and relieved with analgesics. One patient had a superficial infection/cellulitis relieved by antibiotics and another patient required surgery to remove soft tissue necrosis. Clinical and imaging follow-up were available in all 3 patients ranging 1 to 64 months (mean 28 months). No recurrences were encountered.  

CONCLUSION

RFA is a minimally invasive alternative method in the treatment of selected cases of desmoid tumors.  

CLINICAL RELEVANCE/APPLICATION

RFA is a promising alternative to surgical resection in the treatment of desmoid tumors.
