CONCLUSION

Three versions of EIR implementing based on the purpose of image information utilization provides a feasible, practical and potential solution for communication and application of large volume image data in enterprise PACS.

BACKGROUND

To explore a total solution of communication and application for large volume image data in enterprise PACS using electronic image record (EIR). An enterprise PACS covered two million population and two administrative districts in Shanghai was installed in May 2008. 2900 CT studies performed within two months were divided into two groups: routing study (RS) of 1768 cases and large volume study (LVS) of 1132 cases including high resolution and three dimension imaging, which counts to 82.8% of total image capacity in this study. The Electronic Image Record (EIR) was implemented as three versions according to purpose of application : Diagnostic EIR(D-EIR) provides all series images for diagnosis of radiologists, clinical EIR(C-EIR) provides routing axial series image 5mm thick or more for the reference of clinic doctor and public EIR(P-EIR) provides report only for public users. All the images in 1132 cases with LVS were processed by two protocols: D-EIR consist of all series of images and C-EIR consist of post-processing series of images including axial images 5mm thick or more, MIP, MPR and VR images.

EVALUATION

The quality, volume and transfer time of D- and C-EIR were accepted widely by radiologists and clinic physicians. Among the LVS group, the capacity of image file in LVS group is 87.2% (95.1 to 67.4%) reduction and transfer time is 84.7% (67.4 to 95.1%) reduction.

DISCUSSION

Statistical difference exists between C-EIR and D-EIR in image capacity and transfer time. The routine imaging group remains the same in image capacity and transfer time.
