Analysis of Dual Source CT Coronary Angiography of Congenital Heart Disease on Childern

PURPOSE

To analyze the image quality of coronary arteries by dual-source computed tomography(DSCT)in congential heart disease(CHD)childern.

METHOD AND MATERIALS

Image quality of coronary arteries were scored by two radiologists in four categories. More than 3 point was considered to be good image quality and with diagnostic value.Score differences among 3 groups of baseline heart rate levels,3 groups of age level and 4 coronary branches were analyzed by using F test and SNK test. Multiple linear regressions were used to analyze the predictors of age,heart rate, tube voltage,tube current, pitch, contrast volume,contrast injection rate with the coronary image scores.

RESULTS

A total of 72 consecutive patients were enrolled (average age of 1.5 years,42 days to 16 years old). Coronary artery branches of 71.2% (205/288) were evaluable. Image quality of left main coronary artery (LM,score of 3.64±0.49),left anterior descending (LAD,score of 2.97±0.40), right coronary artery (RCA,score of 2.82±0.70),left circumflex (LCX, score of 2.76 ± 0.73) were in a descending order (F=29.00,P<0.0001). Image score of age groups of 0-1 years old group(2.80 ± 0.65),1-3 years old group(2.81 ± 0.74),3-14 years old group (3.58± 0.52) were in a ascending order (F=20.16,P<0.0001).3-14 years old group was statistic different from 0-1 years old group and 1-3 years old group,there were no difference between these two groups. Image score of heart rate groups of <100 bpm group(3.54±0.56),100-150 bpm group(2.86±0.70) and >150 bpm group(2.81±0.63) were in a descending order(F=16.72,P<0.0001).(<100) bpm group was statistic different from 100-150 bpm group and >150 bpm group,there were no difference between these two groups.There were different impact factors predicted image quality of LM, LAD, LCX and RCA.Age(r=0.013,P=0.008)and contrast volume(r=0.292,P=0.011)had positive correlation with LM.age(r=0.509,P<0.001)and voltage(r=0.292,P=0.004)had positive correlation with LAD,Heartrate(r=-0.179,P=0.037)had negative correlation with LAD.Age(r=0.063,P<0.001)had negative correlation with LCX.Age(r=0.486,P=0.011)and Voltage(r=0.220,P=0.035)had positive correlation with RCA.

CONCLUSION

DSCT could clearly visualizes coronary arteries of CHD children over the age of 3 or heart rate less than 100 bpm. Limitations of  DSCT coronary angiography for CHD children were the relatively young age and fast heart rate.
