To demonstrate the MRI detected substrates for classical intractable epilepsy cases with video- EEG correlation.
Ten classical intractable epilepsy syndromes are demonstrated with video- EEG and their MRI correlates discussed. They include dominant mesial temporal sclerosis, non- dominant mesial temporal sclerosis, temporal neocortical seizures, frontal ( motor, supplementary motor, mesial frontal/ orbito frontal), parietal and Occipital seizures. In addition a case of gelastic seizure due to hypothalamic hamartoma and epilepsia partialis continua due to Rasmussen’s encephalitis were also described.
Video- EEG often provides valuable information about the possible ictal zone and the neuroradiologists should be aware of the specific features to be looked in MR imaging.
Advanced imaging should be tried in all such cases in order to demonstrate the possible substrate as it is very essential before possible epilepsy surgery
