to correlate the apparent diffusion coefficient (ADC) value of pharyngeal squamous cell carcinoma with morphologic and histologic prognostic factors.
This study included 45 patients (29 male and 16 female mean age is 61ys) with pharyngeal squamous cell carcinoma. Routine MR imaging and diffusion weighted MR imaging were done on a 1.5tesla MR unit (Symphomy-Siemens). Diffusion MR imaging was done using a single shot echo planar imaging (EPI) with a diffusion-weighted factor, factor b of 0,500 & 1000 sec/mm2. The ADC map was reconstructed with calculation of apparent diffusion coefficient (ADC) values. The ADC value correlated with the morphologic prognostic factors (tumor size, nodal state, distant metastasis), histologic grade and immuno-histochemistry biomarkers (Ki-67, p53). Pearsons correlation test were used to determine the strength of relationship between the ADC value and prognostic factors.
The mean ADC value of squamous cell carcinoma was 1.17±0.12X 10-3 mm2/sec. There was significant different in the ADC value between non and well differentiated tumors (P<0.003). Lower ADC value was associated with established predictor of poor prognosis. It was associated with larger tumor size, higher histologic grade, presence of lymph nodes, lower Ki-67. The ADC value of squamous cell carcinoma correlated with the size of tumor (P<0.002), associated lymph nodes ( P<0.003), tumor grade (P<0.04), Ki-67 (P<0.03) and p53 (p<0.05).
Apparent diffusion coefficient value of head and neck tumor is a new prognostic parameter of squamous cell carcinoma of the pharynx that correlated well with morphologic and histologic prognostic parameters.
Apparent diffusion coefficient value is a new non-invasive prognostic parameter of pharyngeal squamous cell carcinoma that may identify highly aggressive tumors.
