Tomotherapy for Breast Cancer: Contouring and Treatment Volumes Definition

PURPOSE

To describe and define the breast and lymph nodes treatment volumes and their contouring as a principal part of dosimetric planning for tomotherapy treatment.

METHOD AND MATERIALS

The breast, axillary, supraclavicular (SCN), infraclavicular (ICN) and internal mammary chain (IMC) lymph node volumes, planning treatment volumes (PTV), Rotter’s node, and brachial plexus were contoured for 10 patients from 3 radiations oncologists. The contouring procedure was initially executed by 3 physicians and then after discussion and definition of different volumes, and after corrections, it was done by one radiation oncologist. The prescribed dose was 50 Gy in 25 fractions in all treated volumes. Dosimetric planning was done using the standard procedure for all patients. Then the dosimetric procedure was executed for Tomotherapy treatment. The PTV coverage of standard technique and tomotherapy are compared. The heart and lung volumes included in the PTV are evaluated. The dose to the brachial plexus was also evaluated.

RESULTS

All 10 patients were contoured and different definitions and modifications in the PTV were realized to obtain homogeneity in the PTV and to decrease the heart and lung dose. After discussions, the PTV and standard protocols of contouring have been developed. Anatomical, radiological and clinical volumes were compared.

CONCLUSION

The volume definitions and the definitive results of this study will be presented at the meeting.
