PURPOSE/AIM

1. To show the signal intensity characteristics of the colorectal wall layers with high-resolution MR imaging. 2. To demonstrate the usefulness of high-resolution MR imaging for evaluating the mural invasion and lymph node metastasis in early colorectal carcinomas.

CONTENT ORGANIZATION

1. Normal Colorectal Wall: High-resolution MR images clearly depicted the normal colorectal wall as consisting of four to seven layers, which corresponded well with the histologic layers. 2. Mural Invasion of Carcinoma: In 90 (94%) of the 96 carcinomas, the depth of mural invasion at high-resolution MR imaging correlated well with that at histopathology. However, the stage at MR imaging was higher in five carcinomas than the histopathologic stage, and lower in one carcinoma. 3. Lymph Node Metastasis: In 37 (39%) of the 96 carcinomas, lymph node enlargement was depicted at high-resolution MR imaging, and size and signal intensity were useful for differentiating metastasis and reactive enlargement.

SUMMARY

1. High-resolution MR imaging clearly depicts the layers of the colorectal wall. 2. It has high diagnostic accuracy for evaluating the mural invasion and lymph node metastasis in early colorectal carcinomas. 3. High-resolution MR imaging appears to be valuable in preoperative histopathologic staging and lymph node metastasis evaluation in early colorectal carcinomas.
