Comparison of 64-slice MDCT and Two-dimensional Transthoracic Echocardiography for Left Ventricular Ejection Fraction and Regional Wall Motion Assessment in Heterogeneous Patient Populations

To assess left ventricular (LV) ejection fraction (EF) and regional wall motion using 64-slice MDCT in comparison with two-dimensional transthoracic echocardiography (2D-TTE) in heterogeneous patient populations.
A total of 97 patients (22 acute myocardial infarction[AMI], 17 chronic myocardial infarction[CMI], 41 angina pectoris[AP], 7 valvular heart diseases[VHD], 10 atypical chest pain without significant coronary stenosis[ACP]) underwent 64-slice MDCT and 2D-TTE. LV-EF and regional wall motion abnormality(RWMA) assessment by MDCT were compared to 2D-TTE. RWMA assessment was based on the 16-segment model on a 4-point scale.
LV-EFs by MDCT and 2D-TTE were similar (58.5 ± 11.5% vs 56.7 ± 11.1%; P=NS) and highly correlated (r=0.88; P 65bpm, there was no differences of EF and RWMA between the two groups. When correlation of EF and RWMA was assessed by MDCT and TTE each disease, the results are followed; AMI (EF [r=0.80], RWMA [r=0.53]); CMI (EF [r=0.84 ], RWMA [r=0.46]); AP (EF [r=0.73], RWMA [r=0.69]); VHD (EF [r=0.96], RWMA [r=0.65]); and ACP (EF [r=0.70], RWMA [r=1]).
64-slice MDCT provides comparable results to TTE for global left ventricular functional assessment among heterogeneous patient populations. But the regional wall motion assessment correlation is limited between the two modalities in patients with myocardial infarction.
The experience with 64-slice MDCT and cardiac function assessment remains limited by small patient numbers and by the rather homogenous patient populations.
