PURPOSE

Evaluation of patient satisfaction and clinical outcome after uterus-preserving operative treatment or uterine artery embolization (UAE) in patients with symptomatic fibroids.

METHOD AND MATERIALS

140 patients with symptomatic uterine fibroids (treated with fibroid enucleation (N=60) or uterine artery embolization (N=80)) were asked retrospective by questionnaire in terms of symptoms caused by the myoma. The changes of the symptoms after myomectomy and UAE were evaluated and compared.

RESULTS

Both myomectomy and embolization leads to a significant improvement of clinical symptoms (reduction of hypermenorrhoea 58 % vs. 77%, reduction of metrorrhagia 78% vs. 77%, reduction of dysmenorrhoea 39% vs. 56%, respectively). A trend to increased improvement of hypermenorrhoea and dysmenorrhoea was noticed after embolization compared to operation. Stationary phase was in the mean 2.3days after hysteroscopy, 4.4 days after laparoscopy, 7.3 days after abdominal section and 2 days after UAE. 85% of the with UAE treated patients would determinate again for a fibroid embolization.

CONCLUSION

Myomecotmy and uterine artery embolization facilitate a sufficient treatment of symptomatic uterine fibroids. Hereby UAE is an alternative therapeutic option to surgery.
