Intestinal Endometriosis: Preoperative Value of Double Contrast Barium Enema

PURPOSE

To evaluate the efficacy of Double Contrast Barium Enema (DCBE) to identify presence and site of lesion in the clinical suspect of intestinal endometriosis in correlation with surgical findings.

METHOD AND MATERIALS

168 Patients underwent surgical resection for pelvic endometriosis with associated intestinal surgery for intestinal endometriosis. We evaluated retrospectively all the DCBE, with particular attention to site, number, extension, extrinsic mass effect, stenosis (annular or plaque-like) and adhesions.

RESULTS

At DCBE we identified 211 intestinal lesions, 198 in the rectosigmoid junction and 13 in the cecum. Lesions determined mild stenosis in 63%, moderate stenosis in 24% and sub-occlusive in 13%. The mean lenght of stenosis estimated with digital imaging resulted 30 mm (range 5-100 mm). Surgery confirmed 194/198 (98%) rectosigmoidal lesions and 12/13 (92%) cecal lesions. Were also found 27 lesions not preoperatively identified, 15 in the left colic segment and 12 in the cecal region. The DCBE sensibility was 88%, accuracy 86%, and positive predictive value 97%.

CONCLUSION

Double Contrast Barium Enema is able to correctly identify the intestinal involvement in endometriosic disease, contributing to a correct preoperative planning.

CLINICAL RELEVANCE/APPLICATION

Double contrast barium enema can demostrate foci of intestinal endometriosis and is recommended as preoperative exam.
