The use of multidetector-row CT for evaluation of thoracic disease and screening for lung cancer has led to a marked increase in the detection of small pulmonary nodules. While a variety of diagnostic techniques, in particular CT and PET, can aid in distinguishing benign from potentially malignant lesions, there will remain after detailed analysis a small but significant number of patients with lesions that require tissue sampling for definitive diagnosis. The purpose of this presentation is to briefly review the scope of the problem and describe the image-guided procedures available for diagnosis including transthoracic needle biopsy and CT-guided localization to aid thoracoscopic resection of small peripheral lesions. As radiologists are now moving into the therapeutic realm by providing image-guided therapy for malignancy, we will review the published data to date on CT-guided radiofrequency ablation for nonresectable lung cancers.
