Intracranial Dural Arteriovenous Fistulas: Usefulness of 3D Time-of-Flight MR Angiography before and after Endovascular Treatment

PURPOSE

To establish the value of 3D time-of-flight (TOF) MR angiography in evaluation of intracranial dural arteriovenous fistula (DAVF) before and after endovascular treatment.

METHOD AND MATERIALS

Fourteen patients (11women and 3men, aged 48 to 79 years) with angiographically proven intracranial DAVFs were included. All patients underwent 3D TOF MR angiography before and after endovascular treatment. Post treatment MRA was performed within 7days after embolization. Subsequent MRA was performed in two patients at 3 and 6 months later. Source images of 3D TOF MR angiography were evaluated by two neuroradiologist. Disagreements were resolved by consensus.

RESULTS

All patients showed high-intensity areas in the venous sinus and 11 patients showed multiple high-intensity curvilinear and nodular structures adjacent to the wall of sinus on source images of 3D TOF MRA before endovascular treatment. In seven patients, DAVFs were completely obliterated immediately after embolization, and the source images of 3D TOF MRA showed disappearance of high-intensity areas in the venous sinus and multiple high-intensity curvilinear and nodular structures adjacent to the wall of sinus. In remaining seven patients with residual DAVFs, high-intensity area in the venous sinus was noted with decreased size. Three of seven patients with residual DAVFs showed decreased size and number of high-intensity curvilinear structure adjacent to the wall of sinus. One patient with residual DAVF showed disappearance high-intensity area in the venous sinus on subsequent 6months follow-up MRA.

CONCLUSION

3D TOF MR angiography (source images) is very useful in the diagnosis of DAVF and the evaluation of treatment.
