Balloon Dilation of Anastomotic Strictures Secondary to Surgical Repair of Esophageal Atresia: Long-term Results

PURPOSE

Purpose: To evaluate the safety, efficacy, and long term results of balloon dilation in strictures secondary to surgical repair of esophageal atresia in 29 children.

METHOD AND MATERIALS

Materials and methods: Twenty-nine children (10 girls and 19 boys) from 1 to 60 months were included in this study. The strictures were more than 50 % at anastomotic site in all patients and the interval between surgical repair and balloon dilation was 1 – 36 months (mean, 6.4). All procedures were performed under sedation or general anesthesia using fluoroscopic guidance. Diameter of balloon was 6-15 mm.

RESULTS

Results: A total of 44 balloon dilation sessions were performed with a range of 1 - 5 procedures (mean: 1.6) per patient. Balloon dilation was successful in 93% (27/29) of the patients. Dysphagia also improved in 93% (27/29) of the patients after balloon dilation. Of the two patients with clinical failure, one underwent surgery and the other underwent covered stent placement. During the mean follow-up period of 3.1 years (range, 1 month-12 years), all 27 patients with clinical success showed no recurred symptom. As complications, transmural perforation occurred in three patients (10 %); two were managed conservatively, but one patient underwent surgery for combined esophageal rupture and tracheo-esophageal fistula.

CONCLUSION

Conclusion: Balloon dilation is an effective procedure with excellent long-term results for the treatment of anastomotic strictures secondary to surgical repair of esophageal atresia.
