To evaluate the feasibility and effect of CT-Guided Iodine-125 Brachytherapy in treating mediastinal lymph nodes metastasis after artificial pneumothorax.
All 33 cases (twenty-one men and twelve women), aged 37~76 years (average, 61.4 years) of mediastinal lymph nodes metastasis were performed CT-guided interstitial Iodine-125 seeds and low-released 5-Fu implantation after artificial pneumothorax. The radioactivity of Iodine-125 seed is 0.6 ~ 0.8 mci. 5-Fu can enhance the lower initial dosage of Iodine-125 seed. All patients were performed under multi-spiral CT (Philips, Mx8000) with optical navigating system(PinPoint), according to the treatment plan of dosage distributing made by TPS(Treatment Plan System, Hokai Co.). The follow-up examinations, including CT enhancing scan and TM (Tumor Marker), were performed in the 1st, 3rd, 6th month after the therapy respectively. Curative effect was analyzed according to the size of tumor: CR: obvious relief (reduced up to 50%), PR: relief (reduced by 25%-50%); MR: slight relief (reduced by 1%-25%); NR: no effect (the size of tumor did not reduced or increased). Curative efficiency: CR+PR.
Artificial pneumothorax of all cases were successful, and a total of 95 mediastinal puctures were performed. No complications had occurred except that 4 cases have little pleural effusion. The procedures took 95 minites ~ 185 minutes(mean, 145 minutes). The number of implanted iodine-125 seeds were 5 ~ 24(mean, 14.25). The implanted low-released 5-Fu dosage was 100 ~ 400mg(mean 250mg). Depending on the distributing of Iodine-125 seeds by CT scan, then decided if the patients needed to be implanted again. One month after the therapy: CR 10 cases, PR 14 cases, MR 9 cases, NR 0 cases. 2nd month: R 15 cases, PR 13 cases, MR 5 cases, NR 0 cases. 3rd month: R 14 cases, PR 16 cases, MR 3 cases, NR 0 cases.The curative efficiency rate of tumor size is 72.72%(24/33) one month after the operation, the 3rd and 6th month is 84.84%(28/33) and 90.91% (30/33) respectively.
CT-Guided Iodine-125 seeds Implantation into the mediastinal lesions after artificial pneumothorax is an effective and safe way for mediastinal metastasis.
