Preoperative Evaluation of Neurovascular Compression in Patients with Facial Neuralgia by Use of High-Resolution Virtual MR Endoscopy

PURPOSE

To assess the value of high-resolution virtual MR endoscopy reconstructed from three-dimensional fast imaging employing steady-state acquisition (3-D-Fiesta) images for the visualization of neurovascular compression in patients with facial neuralgia.

METHOD AND MATERIALS

Thirty consecutive patients with facial neuralgia underwent preoperative 3-D-Fiesta imaging.Virtual MR endoscopy of nerves and vessels of the cerebellopontine angle was performed in advantage workstation.We compared the virtual MR endoscopy with intraoperative findings.

RESULTS

Virtual MR endoscopy reconstructed from 3-D-Fiesta imaging clearly delineated the facial nerve and adjacent vessels.Virtual MR endoscopy exactly showed the spatial relationship between the facial nerve and causative vessels.The responsible arteries were identified from virtual MR endoscopy,which closely accorded with the microscopic operative view.

CONCLUSION

Virtual MR endoscopy reconstructed from 3-D-Fiesta imaging are very useful for identifying the responsible arteries and in deciding whether to perform surgery in patients with facial neuralgia.
